Free-and-clear property cash-out
A property with no mortgage can still be refinanced conventionally as cash-out.
Fannie
Fannie expressly permits a cash-out refinance to place a new first mortgage on a property the borrower owns free and clear, subject to cash-out ownership, LTV, eligibility and underwriting requirements.
Freddie
Freddie also defines refinance transactions to include a new mortgage on a property previously owned free and clear and applies its cash-out requirements when equity is extracted.
Useful distinction: there is no existing first lien to season, but the borrower still must satisfy the applicable ownership/title history requirements unless an exception applies.

Refinance classification matters. A transaction that falls outside limited/no-cash-out rules may become cash-out even when the borrower receives little or no cash personally.
